RIVERDALE, NEW YORK – The Methodist Home for Nursing and Rehabilitation is excited to announce a new partnership with Northwell Health that will provide telehealth services for their skilled nursing facility patients.

The TeleSNF program will enable the Methodist Home to provide an even higher level of care and service to at-risk patients while reducing hospitalizations. When an at-risk patient has a worsening condition, facility staff will be able to use mobile technology to facilitate a telehealth consultation with the patient, doctor, and family members.

A team of remote critical care nurses, intensivists, and hospitalists are available 24/7 as part of this program. Patient vitals can be monitored and transmitted to Northwell Health staff, who then advise the Methodist Home nurses about the patient’s treatment plan.

This technology gives the entire medical team the ability to triage a patient from the comfort of the Methodist Home, instead of the patient having to venture out for a doctor appointment amid the COVID-19 pandemic.
