Area Medical Partners
We are connected to the Northwell Health TeleSNF Program which offers our nursing team after-hours access to critical care physicians. This technology allows providers to evaluate, monitor, and treat patients via a video conference, often preventing an urgent patient transfer to the hospital.
Our Medical Director is geriatrician Dr. Paul Cavaluzzi. We also work closely with Montefiore Medical Center, New York Presbyterian Hospital, Jansen Hospice & Palliative Care, Allcare Orthotic & Prosthetic Services, Language Fundamentals, and other local providers as patient needs arise.
