Subacute rehabilitation plays a crucial role in helping individuals regain strength and independence after an illness, injury, or surgery.
At the Methodist Home, our subacute rehabilitation program is designed to provide thorough and personalized care to our short-term guests while we guide them on the path to recovery.
So, what do you need to know about subacute rehabilitation when seeking a short term solution for yourself or your loved one?
Understanding Subacute Rehabilitation
Subacute rehabilitation refers to a level of care that falls between acute inpatient hospital care and traditional long-term care. It is designed for individuals who require additional medical supervision and therapy services following a hospital stay but do not need the extensive care provided by a long-term care facility.
Also referred to as short term rehabilitation, it focuses on improving functional abilities, promoting independence, and facilitating a safe transition back home.
Our Rehabilitation Center
At the Methodist Home, our 2,000-square-foot rehabilitation center serves as the headquarters of our subacute rehabilitation program. We offer 40 private rooms equipped with full bathrooms to ensure privacy and comfort for our short-term guests.
Our state-of-the-art “Activities of Daily Living Suite” allows patients to practice essential everyday tasks like cooking and laundry so they can regain confidence in their abilities before returning home.
Comprehensive Therapy Services
Our subacute rehabilitation program encompasses a wide range of therapy services tailored to meet the unique needs of each individual. This includes physical therapy to restore mobility and strength, occupational therapy to focus on daily living skills, and speech therapy to address speech, language, and swallowing difficulties.
The experienced in-house therapists at the Methodist Home work collaboratively with our nursing and other departments to develop personalized treatment plans and guide patients on their journey to recovery.
Specialized Rehabilitation Programs
In addition to general rehabilitation services, we offer specialized programs to address specific conditions and treatment needs.
Our orthopedic and joint replacement rehabilitation program focuses on restoring function and mobility following joint replacement surgery, while our stroke recovery program provides comprehensive rehabilitation services guided by a physiatrist.
We also offer cardiac rehabilitation for individuals recovering from heart-related conditions, and amputee rehabilitation to support individuals adjusting to limb loss and prosthetic fitting and education.
Patient-Centered Care Approach
At the Methodist Home, we prioritize patient-centered care and focus on maximizing functioning and independence for each individual.
Our interdisciplinary team collaborates closely to develop personalized care plans that address the physical, emotional, and social needs of our short-term rehabilitation patients.
Whether it’s physical therapy to improve mobility or nursing services to manage pain, our goal is to help patients achieve their treatment goals and return home safely.
The Intake and Rehabilitation Process
Our intake and rehabilitation process is designed to ensure a seamless transition for our short-term rehabilitation patients. It begins with a referral from a physician or case manager, followed by a comprehensive review of the patient’s needs by our interdisciplinary team.
Once admitted, patients receive personalized care and support from our dedicated staff, including therapy sessions, nursing care, and coordination of consultations with specialists as needed.
When therapy and nursing goals have been met, our care coordination team facilitates a smooth discharge process to ensure a successful transition back home.
